The Orléans Health Hub (OHH) is breaking ground on a new kind of patient experience. Their innovative model brings together specialized and community healthcare services to provide everyone from newborns to seniors with high-quality care close to home, focused on their needs and in both official languages.
The OHH team collaborated with Calian to realize their vision to:

• Enable community partners to collaborate in a multi-service care hub
• Ensure secure, convenient access to multiple care providers in one place
• Extend to easily add new care providers and customize services to meet local needs

Patient-Centric Model for Care

Orléans Health Hub combines three hospitals and six community-based service providers to improve coordination of care and services in a highly accessible, bilingual system that reflects the community. The Hub brings together a wide range of specialized and community healthcare services under one roof for a seamless, connected experience for patients.

“We needed a platform that provided flexibility and agility to meet ever-growing and changing demands. And with the Calian® Corolar™ platform, we now have that,” says Philippe Marleau, Vice-President, Quality and Patient Support Services and CIO at Montfort Hospital.

The multiservice care hub model enables a seamless flow of information and care centred on the patient’s needs, in-person or virtual. Geriatric patients, for example, can access memory care, nutritional guidance, diabetic services—all in one place. “I think that the fact that you can combine multiple services for the same client in one place… that’s just beyond value,” says Dr. Frank Knoefel, Physician, Bruyère Memory Program, OHH.
